RMS AFTER SCHOOL ATHLETICS

In order to participate in afterschool sports, all students must turn in all necessary paperwork to the coach or Athletic Director before tryouts

1. Physical- completed by your doctor once per year.  If you had one last year, it is good for 365 days after the date of your appointment, but must be on file with the Athletic Director. Check with Ms.K if you aren't sure.

2.  Concussion Form (completed once per year)

3. Parental Permission (completed for EVERY sport you play)

4. Anti-Steroid Agreement (completed once per year)

5. Sudden Cardiac Arrest Info Sheet (completed once per year)

RMS student athletes must maintain at least a C grade point average with no D's or F’s in any class, and treat all community members with respect.

Please turn in all paperwork together at the same time.

If you plan on trying out for more than one sport, you can turn in your parent permission fore each sport you plan to try out for early.

It is ALWAYS a good idea to make copies of this paperwork for your own records.
